We are currently looking to recruit highly motivated and experienced Nurses with advanced nursing skills who enjoy working as part of a team, and are keen to embrace a new way of working and want to be included in the development of delivering care in a contemporary fashion whilst part of the NHS working in Tower Hamlets within The Community Health Team.
The Community Health Teams work under an alliance model in partnership across Barts Health, East London Foundation Trust, Social Care, GP care group and Voluntary services. By bringing all these organisations together, not only can we improve the care we give to the patient, but we have an excellent opportunity to develop the skills of our team and this allows staff to gain professional development which is multi-faceted by crossing organisation and professional boundaries. These teams are multidisciplinary, integrated teams comprising of nurses, physiotherapists, psychologists, occupational therapists, care coordinators, specialist nurses, social care, health support workers and administrative staff. We also work closely with Hospice and Primary care staff.
The main aim is to work together to achieve flexible, proactive and patient-focused care for all adults. These teams enhance the links with primary care and other stakeholders, through better access to services and clear lines of communication between practices and CHTs. The teams support the management of known patients at high risk of admission aiming to reduce unnecessary admission to hospital, as well as support discharge out of hospital to reduce length of stay. We are currently a Vanguard site and this means we have the ability to invest in new care models which relate to being a multi-speciality provider.
We pride ourselves on focusing on continuation of professional development and we second nurses for further education including the District nursing qualification. We welcome any nurses with a keen interest in community nursing and a shared vision of giving the best possible care to patients. We have a have a team of practice development facilitators to work with our staff to ensure they continue their learning and gain new skills. We have a dedication as a Trust to support our staff and have a passion for developing autonomous practitioners with a wide range of nursing skills. We welcome applications from nurses who are experienced and newly qualified. We facilitate secondment to District Nurse training and actively invite nurses to develop specialist community clinical skills and personal development through our education and training department.
Posts are part time or full time, and applicants will be required to work a full range of shifts, which cover working over the full week and include Bank Holidays.
