Job overview
The Community Children’s Specialist Nursing Team is a key element within Northamptonshire Healthcare Foundation NHS Trust. The remit of the Long term conditions team, community children’s nursing service (CCN) is to provide nursing assessment and interventions to children with a defined nursing need including palliative and End of Life Care.
You will work as an autonomous practitioner providing nursing care closer to home, or at home depending on the needs of the child and young person. The care will include managing children who are technology dependent, enteral feeding, requiring CVAD care including oncology, oxygen dependent, cardiac monitoring and complex care needs. There is also an on call rota for the service and you will be expected to be part of this.
There is one permanent position and one 12 month fixed term position available.
Main duties of the job
The role will encompass a range of clinical interventions including assessment, intervention and case management, working within multi-disciplinary and locality based model of care. Post holder should act as an advocate for children negotiating care requirements with a variety of partner agencies. Safeguarding is a key element of responsibility for all staff within the team and the post holder should adhere to protocols at all times and support staff members with Safeguarding issues:
- To work as part of the team to provide nursing assessments, interventions and nursing care to children, young people and families on the caseload.
- To work as part of a mixed skill team in partnership with children and families/carers to implement individualised care plans to meet medical and nursing needs.
- To carry a caseload and act as lead professional as appropriate.
- To provide leadership and act as team leader if required, advising schools and partner agencies.
- To support other grades of staff in gaining knowledge and skills to enable high quality care for children is maintained.
- To lead and/or participate as required/requested in team activities.
- To participate as part of skill mix team and within sphere of competence in Palliative and End of Life care
- To be an advocate for the child, young person and family to ensure their holistic needs are met.
