JOB: Clinical Triage Practitioner Band 6 -Swindon

The team are looking for 3 band 6 clinical triage practitioners.

They will manage calls from Healthcare Professionals across BSW.
They will be triaging referrals and moving patients into the correct pathway i.e. hospital at home, integrated neighbourhood teams, urgent care response etc.
They will be working through the referrals on the waiting list.
They need to have excellent IT skills, expereince in Systmone, have a smart card and the ability to triage in a multidisciplinary skills set. There are 3 bases; Swindon, Peasdown & Greenway. Hybrid working may be offered.
Shift times are the following - 8am - 8pm, 8am - 4pm or 12noon - 8pm
