The purpose of Public Health Nursing is to improve health and social wellbeing through taking a population wide perspective for Public Health improvement, focusing on tackling inequalities. This is achieved through the responsibility of working with individuals/families/groups/partner agencies to identify health needs, raise awareness of health and social wellbeing and through enabling/empowering people to improve their own health.

The postholder will take a lead role(where appropriate) within a Public Health nursing team skill mix team, be accountable for a caseload, be a clinical expert, provide public health expertise, and be a key member of the integrated Primary Care Team and other multi-disciplinary groups.

The postholder will fulfil the Community Practice Teacher role for student Public Health nurses on community placement.

Aligned/Linked to a local GP practice/s, you will be responsible for the provision of Public Health nursing services with delegation, if appropriate, to meet the needs of the practice population in a variety of settings, e.g. client's home, health clinic, community centre, schools etc.

Public Health nurses provide service to families across the spectrum from routine mandatory care, e.g. Assessment /HPI allocation /immunisation, to complex, vulnerable situations e.g. Child Protection, domestic abuse, substance misuse etc.

Practitioners also work collectively within multi-disciplinary/agency teams to identify, plan for and address the wider public health needs of the Locality Health Improvement Agenda and may lead in selected areas of priority or special interest within the locality.

Public Health nurses are autonomous practitioners who practice within the legal and ethical framework of the NMC and national legislation, and who have overall responsibility for the supervision/management of a health visiting caseload, being accountable for the quality of care delivered through robust assessment, referral and communication processes and implementation of evidence based practice interventions to address need.

Act up for Team Leader as required.

Registered Nurse with valid NMC Registration.

Registered Health Visitor/ Specialist Practitioner Qualification in Public Health Nursing.

Possession of the Field Work Teachers Certificate/Community Practice Teacher.
