Respiratory Nurse Specialist - Community Respiratory Team

You will provide support and timely intervention to patients with chronic respiratory disease in the community. You will prevent unnecessary or unscheduled acute hospital admissions and provide specialist respiratory care to patients with Chronic obstructive pulmonary disease (COPD). You will provide clinical expertise and support to healthcare colleagues working in respiratory care field. You will support the early discharge of COPD patients from hospital, and will lead on the development of the use of ACPS with the team as a whole, provide leadership, guidance and peer support in the use of ACPs, and lead on the data collection, analysis and dissemination of this work. You will link with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists. It is essential that you have a Degree or Diploma in nursing and should be currently registered with the NMC. You should have relevant post graduate experience including respiratory experience. The shift pattern for this post is Monday - Friday 8.30am - 4.30pm. ( Saturday / Sunday working may be required as part of ongoing service development) . A current Driving Licence is required for this post.
