Nurse Team Leader - Biopolar Hub
An opportunity has arisen to recruit to a temporary Band 7 Nurse Team Leader post on a part time basis to develop and work partnership with Bipolar Scotland to develop a Bipolar Hub. The hub would act a resource for the provision of a range of supports and interventions to complement on-going care provided within CMHT's for people with a diagnosis of Bipolar Disorder. It is designed to act as a 'one stop shop' resource in order to ensure up-to-date information, advice and support is readily available for individuals with Bipolar Disorder. It would support individuals take a more active role in their care and the development of self-management skills as well as placing an emphasis on recovery and greater patient autonomy. It is envisaged that the implementation of the Hub could translate to a reduced utilisation of existing services within CMHTs contributing to increased capacity within existing services.
An initial test of change will be limited to the South Glasgow and East Renfrewshire providing a service to patients within five Adult CMHT's. The services offered will include, peer support groups run by Bipolar Scotland
- Structured therapeutic work in the form of Group psycho education groups for patients and carers
- Physical health checks , lifestyle advice including on diet and support for smoking cessation, pregnancy and family planning advice
- Access to pharmacists for advice on medication and side effect monitoring and medication concordance
- Advice and information about additional resources such as housing, benefits, carers support and employment supports.
Applicants should be a Registered Mental Nurse with current NMC registration and should have significant experience preferably in community services at Band 6, together with evidence of continuing professional development.
