About The Critical Care Outreach Team (CCOT)

The CCOT is a multidisciplinary team of experienced critical care practitioners that assist in the care of patients who are acutely unwell, deteriorating or at high risk of deterioration. This is achieved by supporting, educating and working with ward staff and the multidisciplinary team to optimise the quality and appropriateness of acutely and chronically ill patients’ treatment, care and experience. The CCOT members are also part of the hospital wide resuscitation service providing clinical response to peri/cardiac arrest and massive haemorrhage calls. The CCOT at LBH has an active role in assessing the standards of care across the hospital, reporting findings to the Governance Team and action plan delivery to improve safety and care standards.

Job Description
As the Critical Care Outreach Team Lead, you will contribute to a 24 hour, 7 days a week service provision.

You will be responsible and accountable for your own practice and for ensuring the delivery of a high quality service at all times. You will be the line manager of the members of the team maintaining the image of a role model and ensuring key objectives for the department are achieved in line with national guidance.

As the Resuscitation Link for the London Bridge Hospital Campus, you will lead the resuscitation service delivery, accurately monitor activity, report to and liaise with the Governance Team and represent the London Bridge Hospital in the Operational and Corporate Committees. You will be responsible for the delivery of a structured unannounced simulations programme, identifying learning outcomes and developing action plans accordingly.

Duties & Responsibilities
To meet the Department of Health’s objectives for critical and acute care, and to ensure there is a strategic approach for the delivery of Critical Care Outreach Services locally, which reflects that of the National Strategy and those of the Critical Care Networks.
To underpin Critical Care Outreach practice and service development with the best evidence where it is available.
Deliver intensive care skills to patients with, or at risk of, critical illness receiving care in locations outside the Critical Care Unit.
Enable discharges from the Critical Care Unit by supporting the continuing recovery of discharged patients on wards.
Assess, plan, implement and evaluate care to meet the patient and their family’s needs.
Monitor and review the effectiveness of interventions with the patient and colleagues and modify this to meet changing needs and established goals of care.
Support staff, including those rotating into the team from the Critical Care Unit, to develop their knowledge and skills through acting as a mentor/facilitator and assessor.

Skills and experience
NMC Registered General Nurse.
Advanced Nurse Practitioner background
ALS provider
ITU course completed - 60 credits
Significant experience within a critical/acute care nursing environment
Prior experience in a senior or leadership nursing role is essential
