Based within a community mental health team within GMMH, you will be supporting social workers and mental health professionals across Greater Manchester, the North West of England and beyond. Care co-ordinating as required in line with the Trust Care Programme Approach (CPA) guidance and policies. You will report to the Team Manager for day to day management and will always aim to promote the independence and well-being of adults with mental health needs; assist social inclusion and valued community presence and enable the widest possible expression of chosen lifestyle.
We have a long-term placement opportunity (minimum of 6 months) available for a Band 6 Social Worker or CPN post at Greater Manchester Mental Health Trust, where you will undertake formal mental health and social care assessments of new clients within own caseload, ensuring that service users and carers receive clear and accurate information on:
- How to access the service around the clock
- Access to Records
- Their own Assessments and Care Plans
- Mental Health conditions and medication
- Service Publicity, including what they can expect from the service
- Community Resources
- Complaints and compliments procedures
- How to access Advocacy Services
Teams provide assessment and treatment to those patients experiencing a wide range of mental health problems including; depression, anxiety, psychosis, dementia, delirium and a wide range of specialist mental health and substance misuse services. They also provide Health and Justice Services in a number of custodial settings.

- Degree or equivalent in Social Work
- Hold a current HCPC
- Possess skills as an experienced/be an approved mental health Social Worker, CPN or AMHP
- Actively participate in clinical quality assurance and learning programmes.
- Have a minimum of 6 months experience within Adult Services
