Hospice UK is the national charity working for those experiencing dying, death and bereavement. We work for the benefit of people affected by death and dying, collaborating with our hospice members and other partners who work in end of life care. Our hospice members influence and guide our work to put people at the centre of all we do.
In 2018 Hospice UK, introduced Project ECHO (Extension of Community Healthcare Outcomes) to the UK to help meet this challenge. An established not-for-profit tele-mentoring project developed in the USA, Project ECHO utilises cloud based networking technology to connect specialist clinical teams (‘HUBS’) and primary care clinicians (‘SPOKES’) to create a community of practice, learning and support with a goal of improving patient care.
To improve care for young people with complex and life-limiting conditions, who are in “Transition” from children’s to adults hospice services, Hospice UK is recruiting for an “ECHO Transitions” Partnership Manager for up to a years contract. By utilising ECHO methodology, you will lead on the creation of a national “ECHO Transition” network with children’s and adults hospices; varied stakeholders within the care sector and existing transition services with the aim of facilitating effective partnership working and to hear the voices of the young people in transition to drive the change that is needed in this important field of hospice care.
Reporting to the Senior Clinical and Quality Improvement Lead and working closely alongside the ECHO Project Director, the ECHO Transition Partnerships Manager is the lead for the development and successful roll out of the national “Transition” ECHO Network Group over a three year period. This current position will cover up to one year (Year 2) of the three-year project. The project has achieved the first year ambition to establish three Transition ECHO Hubs, each making progress within their regions, and two of the three hubs fully up and running despite the three-month pause in activity due to the pandemic.
You will bring exceptional project and programme delivery experience from the social care sector combined with excellent communication and influencing skills and understanding of the values of the hospice movement.

Please note that candidates are not expected to have prior experience of Project ECHO and this is not a pre-requisite for the role as full training will be provided to the successful applicant

Occasional travel will be expected as part of this role
