Maundy Relief is a well-established and award winning charity based in Accrington and supporting one of the most disadvantaged communities within the UK. Open to all, our beneficiaries are from all sections of the community and include those experiencing multiple exclusions and the combined effects of mental health problems, addiction, homelessness and poverty. We are committed to developing a Psychologically Informed Environment including recognition of the key importance of Trauma Informed Care. Our work is based on the view that multiple exclusions can best be addressed by case management and a comprehensive and combined set of services. We deliver coordinated talking therapy, community outreach, help with finding and maintaining accommodation and advice and advocacy services.
The Role
The nurse will lead on Maundy Relief’s health offer to clients and will provide open access physical health consultations and advocacy to clients using a biopsychosocial approach. The post holder will be responsible for coordination with statutory health services as necessary and will devise and maintain all necessary clinical records. The nurse will have a casework load and will liaise closely with other project staff to ensure that the nurse service is fully coordinated with our other services. The post holder will deliver an holistic approach to the management of complex mental health conditions whilst retaining focus on the physical health components of their cause and effect.
The Responsibilities
- Lead and develop the health services provided by Maundy Relief
- Develop skills in the understanding and use of a ‘biopsychosocial’ model
- Carry out preliminary biopsychosocial assessments for patients and co-design care pathways with patients, taking into accounts the wider determinants of health
- Deliver appropriate physical health interventions
- Provide health education and advice as required
- Operate open and easily accessed health assessment and treatment clinics
- Support the placements of medical students through mentorship, teaching and supervision.
