Torry Medical Practice in Aberdeen City became a 2C Medical Practice in August 2018.

Our current GP Lead for Torry Medical Practice will be retiring soon therefore Aberdeen City Partnership in conjunction with NHS Grampian are recruiting to the post of a Lead GP for the practice, full-time, shared role/part-time applicants are welcome.

We are looking for a dynamic individual with a vision for change and leadership that is able to guide and shape the developments and tests of change in Torry and share the learning across all City 2C Practices with the support of the Primary Care Team and Aberdeen Health and Social Care Partnership Leadership Team.

This role will work in collaboration with other Clinical Leads across the city, heads of locality, professional leads and other operational managers for Primary and Community Care, to ensure a coherent whole system approach to the delivery of services both within the specific locality and across Aberdeen City.

As an addition to the practice team, Torry Medical Practice has implemented added multi-disciplinary team working in line with the new GMS contract, currently it has 1 FT Physiotherapist and 1 WTE Primary Care Clinical Pharmacist (independent prescriber).

The practice currently has a list size of approximately 7,500 patients. Patient registrations are for AB11 8 and AB11 9 postcodes. The practice population has been increasing steadily since 2008 and the Torry Medical Practice has increased by 107 patients since April 2016. Aberdeen City Health and Social Care Partnership covers the geographical area of Aberdeen City which has a population of approximately 235,000. Within Aberdeen there are currently 29 GP Practices. The City is grouped into four localities based around groups of GP Practices.

The successful post holder will have overall strategic and operational responsibility for the management and delivery of GMS services for Torry Medical Practice. Please see the job description and person specification attached.

The context for this delivery will be the General Medical Service (GMS) contact for General Practice the Strategic Plan for Aberdeen City Health and Social Care Partnership, the National Health and Wellbeing Outcomes and other relevant professionals to ensure the implementation, development and delivery of safe and effective, person-centred systems to enable maximum operational efficiency across Aberdeen.

The Aberdeen City Health and Social Care Partnership is responsible for the planning and delivery of adult health and social care services in Aberdeen City as well as hosting Older People and Rehabilitation hospital based services at Woodend Hospital and Sexual Health Services based at the Community Health and Care Village on behalf of Aberdeenshire and Moray Integrated Joint Boards.
