An opportunity has arisen for a permanent Advanced Nurse Practitioner Band 7 to support the medical management of patients residing in care homes in Falkirk. An Advanced Nurse Practitioner (ANP) is an experienced and highly educated Registered Nurse who manages the complete clinical care for their patient, not solely any specific condition. As a clinical leader they have the freedom and authority to act and accept the responsibility and accountability for those actions. This level of practice is characterised by high level autonomous decision making, including assessment, diagnosis, treatment including prescribing, of patients with complex multi-dimensional problems. Decisions are made using high level expert, knowledge and skills. This includes the authority to refer, admit and discharge within appropriate clinical areas. The successful applicants will provide nursing input to care homes and will proactively take opportunities to educate and empower care home staff to manage healthcare needs as best possible. The purpose of this role is to reduce GP input and encourage the care homes to be self sufficient. Calls from nursing homes vary by home, which indicate that overall demand from care homes could be reduced. It is proposed that most of these visits can safely and competently be done by a nurse with relevant skill set (prescribing, practitioner level practice etc). We are seeking a highly motivated and experienced nurse who will embrace the challenge of this dynamic role. They will provide direct patient management and have sound clinical knowledge and highly developed clinical decision-making skills. All applicants should possess the ability to prioritise fluctuating workloads, remain focused, work across professional boundaries and be flexible. Input will extend to planned care activity such as Anticipatory Care Planning. Nurse input to care homes will proactively take opportunities to educate and empower care home staff to manage healthcare needs as best as possible. Essential Qualifications for band 7: • HEI accredited Independent Prescribing Qualification - V300 • HEI accredited Clinical Examination & history taking course • HEI accredited clinical decision-making module Applicants who possess two of the three qualifications above will be considered for training posts. Training posts will be paid at Band 6 level until successful completion of all essential qualifications. Highly Desirable • Experience of nursing in an Acute and / or Community setting • Evidence of being involved in service development or quality improvement. • Expertise in nursing elderly / frailty assessment skills It is also essential that you have clinical skills in the areas of wound management, catheter care and palliative care including end of life care. To apply for this post it is essential that you are a good communicator and show that you work well within a team and are also able to function autonomously using skills and initiative. This evolving service is proactively involved in new ways of working so it would be an advantage to be motivated and adaptable to change and to have completed further training in communication skills. Ability and willingness to travel throughout the area of service delivery is essential for this post. This is a permanent post and the working hours are 37.5 hours per week. The hours and shift pattern may vary therefore the post holder should be flexible to suit the needs of the service.
