We have an opportunity for a Practitioner of all disciplines to establish robust recommendations of patients' eligibility for NHS Continuing Healthcare funding and joint funding, as a representative of the Local Authority.

You will promote best practice and provide professional advice to care homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to Swindon Borough Council. You will monitor and influence the quality of care that is provided to service users; drawing on previous skills and knowledge to improve the service user experience of care. You will provide professional advice to service users, carers and multi-agency colleagues, including signposting to appropriate services and resources. Your analytical skills should be backed up by excellent written and verbal communication skills. You will have a proven ability to convey sensitive, specialist and complex information. Experience of analysing patient data in accordance with the National Framework for NHS continuing healthcare and NHS funded nursing care (2018) will give you clear advantage over other applicants. You will need to have a thorough understanding of "informed consent" and the Mental Capacity Act and Codes of Practice.

This post will involve working with all service user groups (16+). Experience within Mental Health around Section 117 and the Care Programme Approach (CPA) and or Learning Disabilities would be desirable.


You must be a registered professional (Nurse, Social Worker or Occupational Therapist) with diverse post registration experience including case management. Current professional registration with HCPC/NMC

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act - for the effective performance of a customer-facing role).
