Purpose of the role
In line with the philosophy of the unit the post holder is responsible for the assessment, planning, implementation and evaluation of care for the identified service users, acting as nurse for service users with complex and challenging needs within an enhanced supported living environment.
The post holder will deliver specific interventions on an individual basis, providing effective evidence based care to service users. Communication and liaison with a number of disciplines is essential to promote effective delivery of the clinical care pathway to meet service user's needs.
The post holder will provide support and direction to team members and assist the smooth coordination and running of the unit.
Main duties & responsibilities
Heading Duty/Responsibility Clinical
With the service user, assess, plan implement and evaluate individual support plans within the clinical area's philosophy/approach, for an identified caseload to the standards expected within the service.
Perceives own role as primarily therapeutic.
As a part of the multi-disciplinary clinical team, participate in clinical and CPA reviews, completing all relevant documentation to the agreed standard.
To ensure all clinical activity is recorded fully and in a timely manner, ensuring information is being shared with relevant stakeholders.
To engage effectively with service users, can empathise and has insight into service user's perceptions and is able to negotiate unambiguous boundaries in relationships.
Risk assessment and Management
To ensure robust use of standard risk assessment tools on an initial and ongoing responsive manner within a service user's care and treatment and monitors others' use of same within own area of responsibility.
Supports the service user to take controlled risk, as part of an agreed management plan and in consultation with clinical team members.
To formulate and implement effective risk management strategies which ensure liaison with all relevant clinical team members and operational staff in an individual's care and enabling service user involvement where possible.
To communicate any changes to a service user's risk profile in a timely manner, both operationally to the Service Manager or nominated deputy and clinically to the RC and Care Co-ordinator.
To report adverse incidents and ensure documentation is detailed and up to dat. To ensure risk management decision making is undertaken in a manner which is clinically robust and involves all relevant individuals and is fully documented.
To provide clinical advice and direction when clinical incidents arise and provide effective support for service users and team members, responding to changing needs and priorities as appropriate.
Service Development

To contribute to policy and practice development as identified and proactively seeking opportunities to engage in same
To participate in audit of practice standards and any other audits appropriate to the Service / Organisation.
Professional Development
To receive clinical and managerial supervision and engage in the appraisal process reflecting on own learning needs and developing objectives.
To participate in and develop an up to date personal development plan reflecting on own needs and engaging in development of clear objectives.
To actively participate in and achieve satisfactory abilities in training relevant to the post.
To adhere to NMC code of conduct and maintain and meet requirements for professional registration.
Aware of own abilities and strengths in interacting with others.
Aware of own attitudes towards offending/challenging behaviour.
Recognises own limits and seeks appropriate support.
To provide support and professional advice to team members within own area(s) of expertise and supervise the work of support staff and/or nursing students.
To undertake any other duties associated with this post, as delegated by the Service Manager.
Promotes the work of the Organisation during liaison and contact with other key stakeholders.
