Specialised CAMHS Practitioner/Clinical Nurse Specialist

GLOUCESTER HOUSE:

We are a specialist school, which provides education and therapeutic services for up to 21 children aged 5-14 years who have social, emotional and mental health difficulties (SEMH). Many of the children who attend are from socially disadvantaged backgrounds, and some have experienced severe deprivation or trauma/abuse in their early lives.

Gloucester House provides a unique package for children which combines clinical and educational components. There is an emphasis on whole team multidisciplinary work including identifying which of a range of approaches is best suited to the needs of each child and their family.

The team is comprised of a multidisciplinary CAMHS team and specialist teachers and education staff. The service is lead by a headteacher and clinically lead by nursing.

CHILDREN YOUNG ADULTS AND FAMILIES SERVICES (CYAF):

This is a thriving and growing Directorate that encompasses services for children and young people (0-25) their families, and carers, that spans other Directorates within the Trust. The CYAF Directorate is the largest of these. The clinical services of the Directorate are undertaken by child psychiatrists, child psychotherapists, clinical and educational psychologists, social workers, systemic psychotherapists, nurses and by those training in these disciplines. The Directorate has three specialist units, the Family Drug and Alcohol Court, the Gloucester House Day Unit and the Gender Identity Development Service. The CAYF Directorate provides multi-disciplinary clinical services for a substantial catchment area in addition to offering more specialised services for referrals for a larger area. Recent clinical developments include taking on a substance misuse service for young people in Barnet.

Band 6 Specialist Nurse

Tavistock & Portman NHS Foundation Trust currently host the largest adult Gender Identity service in the UK which cares for over 6000 patients from 18 to end of life. This is an out-patient service currently based in Hammersmith and our small multi-disciplinary team sees people who are in all stages of the care pathway.

The Specialist Nurse plays a vital role together with the Consultant Endocrinologist and Endocrinology Team in monitoring and managing the physical and endocrine needs of the caseload of patients under the care of the Gender Identity Clinic. This work is pivotal in ensuring that hormone regimes are managed effectively and ensuring that physical health needs of patients are carefully managed.

The role entails close working relationships and on-going support of GPs through the implementation of the Shared Care Prescribing Protocol and well as work with other local health services and clinical teams. In partnership with other members of the Multi-disciplinary Team, the role will be required to support holistically the patient’s safe progression through the established care pathway.

No previous gender experience is required and we are keen to hear from registered nurses from all backgrounds who are inspired by this opportunity and believe they have some transferable skills and/or experience. If appointed we will provide you with a personalised training programme and support package to enable you to succeed. You may wish to combine this role with another role and we are also happy to consider this.
