An exciting opportunity has arisen for experienced and highly motivated registered general, mental health and dual registered nurses to work within the recently established and ‘embedded’ South Warwickshire CCG Continuing Healthcare team. These community based posts offer suitably qualified nurses the ability to enhance their assessment and decision‐making skills in a challenging but supportive environment.
With a population of 280,000 and 33 member GP practices, located within the districts of Warwick and Stratford‐on‐Avon, South Warwickshire CCG is an attractive place to work. The CCG has stable and committed clinical and managerial leadership and is focused on improving health outcomes for the local population.
Our recently established ‘embedded model’, whereby staff are hosted by Arden & GEM CSU but operationally directed on a day to day basis by the CCG, provides the opportunity to develop a local service within a CCG which is heavily motivated by quality and patient experience.
South Warwickshire CCG recently carried out a modelling exercise with the current Continuing Healthcare team to determine the team’s capacity to meet current and future patient demand and to provide a robust workforce that is fit for purpose. As a result, the CCG is now recruiting to two new Band 5 Continuing Healthcare Nurse posts.
As a Band 5 Continuing Healthcare Nurse, you will be responsible for assisting with the determination of eligibility for NHS Continuing Health care funding through new and review Continuing Healthcare assessments in accordance with the Department of Health National Continuing Health Care Framework.
You will be responsible for ensuring your patients go through the Continuing Healthcare process smoothly and efficiently, thereby enabling timely provision of appropriate, high quality care. You will communicate and develop strong links with health and social care colleagues to ensure effective multi‐disciplinary working, and develop a strong understanding of local NHS and independent sector service provision in individual’s own homes and in care homes. A Band 6 Continuing Healthcare nurse assessor will provide clinical supervision and mentorship in your role.
Reporting to a Band 6 Continuing Healthcare Clinical Lead the Band 5 Continuing Healthcare nurse will:
- Be highly motivated and keen to learn/develop (training in respect of the framework and its application will be provided as part of induction);
- Be accustomed to a high degree of autonomy on a day to day basis;
- Have excellent verbal and written communication skills, with attention to detail;
- Have excellent people management skills with experience of difficult situations and challenging individuals and the ability to manage these effectively and calmly;
- Be comfortable working with a wide variety of ages and individuals with very different clinical conditions;
- Have a strong value set around quality and patient/carer experience;
- Have excellent time management skills and thrive on working at pace;
- Be comfortable with change.
