An exciting opportunity has arisen to join our busy, multi-disciplinary integrated drug and alcohol service in Croydon.
A key part of your role will be to identify and engage individuals within the borough who have not previously accessed community drug and alcohol treatment services. You will work closely with the alcohol liaison service at Croydon University Hospital, acute health care services and community partners to identify and work with non-engaging individuals who have been discharged from hospital and are living in supported housing schemes.
Another essential part of this role will be to provide the highest standard of assessment, planning, implementation and evaluation of individualised holistic patient care in conjunction with the enhanced outreach practitioner.
You will also provide minor injury wound management in liaison with primary healthcare providers, acute services or tissue viability teams, and supporting individuals engaging with community detoxification.
The successful candidate will need to have a minimum of two years' experience working with substance misuse and mental health/dual diagnosis, as well as a sound knowledge of the substance misuse recovery agenda and Blood Borne Virus (BBV) practice.
The post is five days a week; exact hours may be applicable for further negotiation upon successful appointment.
