Holistically supporting adult service users to enter and move through integrated recovery oriented substance misuse services and increase opportunities for service users to achieve sustainable treatment outcomes within Brent.
Working closely with the team, community safety partnership and Safer Neighbourhood Teams the post holder will contribute to the provision of the full range of services provided within the project and take a lead role in delivering a range of primary health interventions to some of Brent's most "hard to reach" service users, including sex workers, stimulant users, intravenous drug users street drinkers and homeless individuals, through assertive outreach.
The post holder will be the clinical lead for BBV and Primary Care interventions within project as well as the outreach and in-reach provision throughout the borough.
As directed by the project manager, the post holder will work alongside a multi-disciplinary team to deliver a flexible and accessible substance misuse service with particular attention to BBV immunisation and testing, sexual health screening and the provision of primary care interventions.
The post-holder will contribute to the service to ensure that substance users consistently receive high quality services that adhere to best practice guidelines and achieve high professional standards and in addition, will be responsible for contributing to the overall development and performance of the service to ensure that contractual output targets are achieved.
