Newark Health Centre
In conjunction with our clinical commissioning group, Newark & Sherwood has transformed its adult community services into 3 integrated teams and requires an enthusiastic and dynamic District Nurse to join us based at Newark Health Centre. You will be responsible for providing leadership and support to the integrated team and will be an integral part of embedding new service changes within the team and locality.
You must have a BSc (Hons) in Specialist Community Nursing Practice and have significant community experience with the ability to motivate yourself and others.
It is essential you have effective leadership skills and be committed to supporting integrated ways of working.
You must be able to maintain the highest standards of quality care for your patients.
You will be expected to manage a caseload of complex, high risk and supported self care patients. The successful candidate will therefore demonstrate excellent communication and interpersonal skills with the ability to manage and lead change within the community.
You will need to work autonomously within a multidisciplinary environment and be politically aware of service developments. You will be flexible, reliable and approachable.
Health Partnerships will support personal / professional development through annual personal development review and clinical supervision. In house training, Induction and mentorship support is also available.
It is a requirement for this post that you possess a full driving licence and the ability
to travel between locations.
