Community Nurse Specialist
£37,617 per annum pro rata
Full-time, 12 month fixed-term contract

We currently operate six Community Support Services across the UK, located in Stockport, Birmingham, Croydon, Newcastle, Glasgow and Cardiff, with a further three services planned. Each service reflects the needs of the local community it serves and is predominantly commissioned by the appropriate health and social care providers. The service aims to provide a specialist and survivorship service, to support men affected by prostate cancer.
Working closely with the Community Support Service Manager, you will develop and deliver innovative ways of connecting with men and their families, ensuring they have access to specialist information and clinically appropriate support. Using your presence in the community, you will identify gaps in local provision and services, and potential opportunities for specialist nursing care.
You will engage and develop relationships with primary care practitioners, to enhance awareness and understanding and enable compliance with the prostate cancer risk management programme. Working with existing community networks, you will target a priority audience with men who are at higher risk of prostate cancer, including older men and African Caribbean men, as well as those whose life circumstances may mean that they are late in presenting themselves for diagnosis, or have poor access to services.
You will be a Registered Nurse with NMC registration, have a strong team ethic and the ability to communicate effectively with men from a range of backgrounds and health levels. Work placement experience within primary and secondary care is essential, as is a background in running education and awareness raising sessions, to a wide range of audiences. You will also have a sound understanding of prostate cancer pathology, current screening practices and evidence-based survivorship programmes.
