This Long term post is to support the provision of heart failure services for people with heart failure, in particular, the development of community and practice based services for the management of heart failure.
The post holder will:
Support Primary Health Care Professionals in the management of heart failure in the community and delivery of appropriate community based services.
Support the PCT and primary care health professionals in meeting the CHD/Heart Failure NSF Delivery Strategy.
Be a Non-medical prescriber to facilitate the management of cardiac medications and other heart failure related medications and equipment within your caseload and within your sphere of competence.
