Well Being Advisor- RNA (Falls Project)
The ExtraCare Charitable Trust is working along side Milton Keynes Council and the Falls Prevention team, to help individuals in the community who have had falls, have a fear of falling or poor balance/mobility. This is a new and challenging position which be based within the Milton Keynes community on a fixed term contract basis Monday to Friday for 18 months.
You will offer this new and unique comprehensive service to the Milton Keynes citizens by promoting and seeking individuals within the community who have or are likely to fall. You will need to be innovative, creative and resilient in promoting this new and exclusive service to the Milton Keynes community. Although you may receive some referrals from the falls prevention team, GP’s, district nurses and social workers, the main objective of your role will be to seek, promote and find new members for this service in the community. On a daily basis you will be required to provide a home visit service which would involve creating a programme of health and well being. You will be completing health check assessments which include but aren’t limited to: cholesterol, blood sugar, blood pressure, urinalysis readings etc.
The assessments would include key elements such as: mobility and balance, medication review, fitness, home environmental risks, osteoporosis risks and diabetes screening. Following on from these assessments you will be required to formulate a health action plan with a follow up visit; to evaluate the outcome and referring residents to their GP for treatment if required or to the Fitness Instructors to ensure their strength and balance is improved.
What we are looking for:
To be successful within this role, you will be an experienced RNA.
· An experienced community nurse with over 2 years experience gained.
· Own transport is essential
· An exceptional communicator.
· Forward planner, creative in ideas.
· Project management
· Have excellent written, verbal and listening skills.
