e successful candidate will be an experienced Band 6 Rapid Response Nurse Assessor for a working in a Community Healthcare. The role of the Rapid Response Nurse Practitioner is dynamic and the post holder will be proactive in continuing to develop this role beyond the traditional scope of practice of nursing roles, in response to challenging healthcare needs, population demographics and national strategies/guidelines.

Within the role, there will be an expectation to form collaborative working relationships with our partners in acute hospital trusts, working closely with the staff in MAU/A&E and the London ambulance service in the prevention of hospital admission.
