You will be triaging patients over the phone to assess them and referring onto other services as and when required. You will be delivering wellness programmes to individuals and assisting with the health promotion programmes. You must have excellent commuication and interpersonal skill, be flexible and be a good team player. You must be able to demonstrate post registration experience, preferably with elements of case management, Primary Care, Occupational Health or over the telephone.
