Housing 21 is a dynamic not-for-profit organisation committed to improving older people’s lives by promoting independence and choice, through high quality care, support and housing. We have a history of designing and delivering successful innovative services for people with dementia.
Housing 21 is launching a brand new End-of-Life Service for people with dementia living in Bristol and Kent. We’re seeking two committed Nurses, one in each area, preferably dual registered RGN/RMN, with experience of working with people with advanced dementia and end of life care. You’ll need to be an innovative practitioner who is able to work autonomously and who possesses excellent communication skills. You’ll also require an up-to-date knowledge of current palliative and person-centreddementia care.
The main responsibility of these posts will be to provide specialist End-of-Life assessments, support and co-ordination of services for people with dementia so they can remain in their place of choice for longer. You’ll also find innovative solutions in unforeseen circumstances; and establish effective working relationships with professionals across the health, social care and voluntary sectors in Bristol or Kent.
