Community Specialist Nurse
Ealing Community Services are developing a new Intermediate Care Service to provide Rapid Assessment and Response for ambulatory care sensitive patients in the Community who would otherwise be admitted to acute care. The service will operate with extended hours 7 days per week providing up to 72 hours of multidisciplinary care supported by nurses, therapists and the social care reablement service.
You will need to develop to be an autonomous practitioner and prescriber with full physical examination skills. You will have interest and experience working with older people and those with complex & long term conditions. You will work within the Rapid Response Team of senior nurses, therapists and assistants responsible for providing assessment and care in people’s homes and meeting the service targets for providing out of hospital care within a defined budget.
